Request for Service Forms
Listed below are forms available through the AHC-IS:
- Data/File Server Access Request Form
The Data/File Server Access request form is used to add/modify/delete server access. This form should be submitted whenever someone joins or leaves your department, or when someone needs modification to their current server access. Additionally, AHC-IS uses the information submitted on this form to keep our internal database up to date.
- Meeting Maker Request Form
This form is used to add or delete Meeting Maker accounts for people, resources, or locations.
- Fairview Clinical Apps Request Form
This form is used to request the addition or removal of access, or installation of Fairview applications such as PAC’s, FCIS, or Fairview VPN.
- Purchase/Support Request Form
This form has two options. The first option is to inquire about purchasing a new computer, hardware, software. The second option is to add an existing computer to AHC-IS support.
