Report a Violation
There are a number of ways to report legal/policy violations at the University of Minnesota, including those that allow you to remain anonymous.
If you wish to report a violation, please use one of the following options:

Privacy Incident Reporting Form – designed for HIPAA Privacy related incidents, this form is used to report concerns regarding an inappropriate use of a patient’s or research subject’s health information. This form offers the option to remain anonymous.

U Report – a general, external reporting service. Examples of violations to report at U Report include theft; wage, benefit, or hours abuses; discrimination or sexual harassment; misuse of University property or equipment; violation of safety rules; OSHA or environmental abuse concerns; conflicts of interest; NCAA violations; and intentional misuse of the University’s network or computers. U Report offers the option to remain anonymous.
