• Facility Orientation, Training and Access
  • Requesters must be Animal Certified by the IACUC (see IACUC Training and Certification above)
  • Complete ROHP Requirements
  • Be listed on an active IACUC animal care and use protocol
  • Complete the following forms:
    • Initial RAR Clearance & Facility Access (for new staff working with animals)  
    • Application for U-Card Access to Additional RAR facilities (for changes needed after initial clearance above)
      • Complete RAR SPF or NHP training if applicable
    • RAR Training Registration Form
• Animal Transfer Forms
• Animal Move to Different Location
• Protocol Transfer and/or Account String Change +/- Physical Move
