Thank you for taking the time to complete this annual survey. You will be asked to provide essential input on a variety of topics related to your attendance at the School of Public Health (SPH). Your feedback is an invaluable component of ongoing evaluation and improvement.

Although this survey is secure, requiring you to log in with your University of Minnesota internet ID and password, the answers will be de-identified and used only in aggregate.

System requirements:

• Browsers - Internet Explorer or Firefox (Mozilla) for Windows or Mac.
• The survey will take approximately 15 minutes to complete and cannot be saved.
• The survey will only stay active for 60 minutes. Timing out will require you to start over.
