A primary goal of the Powell Center is to provide a model of comprehensive, multi-disciplinary women’s health care that may be replicated in other health systems.  Below are links to the Intake Form and Review of Systems used in the Women’s Health Center.  Because clinical assessment for Intimate Partner Violence (or domestic violence) may be challenging for some providers, we also have included a detailed screening guide from the Family Violence Prevention Fund.
